Show's been making this huge deal over Sam having demon blood in him. First, it was YED's blood, which made him psychic, but with YED's death that power seems to have gone. However, there was still the big "Omg, flail!" that Sam had demon blood in him, making him Different and possiby Evil.
Then there was Ruby's blood, which gave Sam more or different psychic powers. The more blood he got in him, the stronger he was (and also the more different and evil).
So when he was drinking blood to kill Lilith, the amount of power he needed to kill her meant he had to drink a lot of blood which meant he got to be extra evil.
Show's been going on about how Sam can detox from the demon blood, but back whenit was just YED's blood in him, Sam went on about how he had demon blood in him and could never get it out. So that's a no on the detox. Then after Sam sucked Ruby dry (minds out of the gutter!), Castiel believed that that amount of blood and power and the changes they would do to Sam couldn't be gotten rid of with detox. So we have two cases where it says that detoxing wouldn't do anything about the demon blood in Sam - it would just stop the cravings. Which means that Sam is Different, at blood-level. Add to that the "Sam was dead then brought back to life and maybe he wasn't brought back a-ok" doubt, and we have a seriously Different (possibly Evil) Sam.
And we see this when Sam does the anit-angel spell with his blood. So something is different about Sam and it's tied to his blood. Which is why Cas calls him an abomination. And I want to know more about what it is that makes Sam different, and how.
